Welcome to the Maine Bureau of Insurance online data reporting system.

In order to enter information into the system, you will need your company's Maine license number and access code. If you do not have your access code, contact the person listed below for your report.

By accessing these reporting forms, you acknowledge that you have the proper authorization to file these reports on behalf of your company.

Once you click on the Submit button, your information will be submitted to our office and will be unavailable for editing. You will receive a confirmation email with a pdf file containing your data report. If you discover that your data needs to be edited after it has been submitted, contact the appropriate person listed above.
